Seamless care : one team

File consists of two diagrams. The first outlines the present and future states of hospital care for older adults at Fraser Health Authority (FHA) hospitals. The second is a timeline for implementation of a new hospital care plan for older adults across FHA hospitals.

Understanding my stay

File contains two versions of a whiteboard chart (one portrait and one landscape) developed as part of a strategic initiative of the Fraser Health Medicine Program. Chart outlines key information that should be noted for older adults who have been hospitalized and their hospital care providers.

RN competencies model of care

File consists of a diagrammatic representation of the Registered Nurse Competencies Model of Care for older adults, part of the 2011 Provincial Strategy of the British Columbia Ministry of Health, which Vicky Bach played a role in developing. It incorporates another model designed by Vicky—48-6—which presents six care areas for older adults that should be addressed within the first forty-eight hours of their admission to hospital.

File consists of the title page and table of contents of a document that Vicky Bach played a leading role in developing, released in March 2011 by the Fraser Health Older Adult Program. As Vicky states in Series 2, File 2, Item 4, this document was developed in an effort to enhance “acute care by ensuring that nurses understand the impact of hospitalization on the older population and are able to identify their learning gaps and needs.”

Elder friendly e-zine

File consists of two issues—February 2009 and February 2011—of Elder Friendly e-Zine, a monthly publication of the Fraser Health Older Adult Program designed and developed by Vicky Bach to provide useful information to hospital staff in a quick reference format. As Vicky states in Series 2, File 2, Item 4, “[e]ach month highlighted a differed topic related to elderly patients in acute care. Copies were printed and posted throughout hospital sites as well as collected in binders for easy reference.”

Briefing note regarding elder friendly name tags

File consists of a briefing note written by Vicky Bach to Vicki Raw of the Abbotsford Regional Hospital (ARH) Foundation in July 2010 regarding elder friendly name tags for ARH and Mission Memorial Hospital (MMH) staff and the funding needed to procure them.

SBAR instructional records

File consists of a poster which describes a regional initiative to implement the SBAR Tool, created and presented by Vicky Bach at the British Columbia Ministry of Health Innovations in Health Care Best Practices Forum in Vancouver in June 2007. The SBAR (Situation, Background, Assessment, Recommendation) Tool was developed by Vicky while employed with the Fraser Health Authority to improve nurse-physician and nurse-nurse communication in long term care facilities. File includes a one-page handout and two drafts of a small reference card, both of which were created by Vicky and outline the SBAR tool. File also includes a short video recording of Vicky reciting the introduction to an educational CD on the SBAR Tool, the contents of which she developed.

Hot weather safety brochures and poster

File consists of a poster created by Vicky Bach for Fraser Health Authority facilities as part of a campaign of education and information for prevention of heat-related illnesses. File also includes two brochures—one for patients and their families and one for staff— that she created for distribution at these facilities as part of this campaign. The brochure for patients provides “tips and resources for staying cool and safe in extreme heat conditions.” The brochure for staff provides “tips for health care providers and managers.”

Project records

Series consists of records documenting projects in which Vicky Bach played a leading role while employed with the Fraser Health Authority. Included are posters, brochures, diagrams, a reference card, a handout, a briefing note, the title page and table of contents of a document, and a video recording of Vicky.

Toronto General Hospital School of Nursing history, photograph album, and test of hygiene

File consists of a copy of a book—History, the School for Nurses, Toronto General Hospital: commemorating the fiftieth anniversary of its establishment, 1881-1931, edited by Margaret Isabel Lawrence—with the name “Alma Elizabeth Reid” embossed on the cover, likely given to Vicky Bach by Alma Reid. File includes an album related to a 1932 graduating class of nurses containing photographs and a forty-year reunion poem, likely given to Vicky by either Evelyn Hahn or Marjorie Parsons. File also includes a test on the subject of hygiene that was used at Toronto General Hospital School of Nursing on November 25, 1922, likely given to Vicky by one of the interviewees.

Henrietta Alderson transcript, audio cassette, and obituary

File consists of an edited transcript and an audio cassette of Vicky Bach’s oral history interview on February 19, 1993 with Henrietta Alderson, a retired nurse who had trained at Hamilton General Hospital in 1934 (and had served as a faculty member in McMaster’s School of Nursing from 1947 to 1975). File also includes the obituary for Henrietta Alderson published following her death on December 25, 2000.
