Series consists primarily of application forms completed and submitted by candidates for admission to the Training School for Nurses at the TGH 1881-1910 (graduating classes of 1884-1913). Some files also contain materials subsequently added, usually letters from students or their agents requesting information contained in their files.

Sous-fonds consists of applications for admission; student records; Director’s reports and correspondence; registers, day books and procedures manuals; materials relating to the Student Nurses’ Association and to ceremonies and special events connected with the School; education program files and photographs created and maintained by the TGH School of Nursing and subsequently maintained by the TTH Nursing Education and Research Department. Series consists of registers maintained for various purposes. File 1.1.5.1 is a register of graduates of the School 1881-1918, including students’ names and year of graduation. File 1.1.5.2 is a register of graduates of the School 1883-1940, including maiden and married names, whether deceased, and year of graduation. Files 1.1.5.3 and 1.1.5.4 are registers of student grades 1898-1920 and 1920-1935, including students’ and examiners’ names; date; course name; grade. File 1.1.5.5 is a register of students’ attendance 1927-1936. Files 1.1.5.6 and 1.1.5.7 are registers of probationary students 1913-1925 and 1926-1943, including students’ names, age, address; dates of probationary and full admission to the School; notes on students’ health, character, academic performance and potential. Files 1.1.5.8 and 1.1.5.9 are registers 1948-1958 and 1958-1970 documenting use of the School seal, including date, description of purpose of use, and signature of authorized user.

Series consists of photographs documenting various aspects of the administration and student life of the TGH School of Nursing. File 1.1.11.1 contains negatives of graduating class photographs 1888-1974: negatives for 1889, 1892 to 1899 are missing. File 1.1.11.9 contains portraits of TGH School of Nursing Superintendents Mary Agnes Snively, Jean I. Gunn, and Mary E. Macfarland.

Sub-series consists of reports prepared by the Superintendent/ Director of the TGH School of Nursing and presented at the School’s annual convocation ceremonies for graduating students in May or June and/or submitted to the TGH Superintendent/ Executive Director as an academic year-end report, usually in September. Graduation reports 1 to 7 and 12, for the years 1882 to 1888, and 1893 are missing. For each of the years 1889 to 1906 a single report was prepared and was presented at graduation. For each of the years 1907-1921, 1923-1925 two reports were prepared: the first was presented at graduation in May; the second was submitted to the TGH Superintendent. For each of the years 1926 to 1970 a single report was prepared and was presented at graduation. For each of the years 1971 to 1973 a single report was prepared and was submitted to the TGH Executive Director.

Reports contain general information pertinent to the administration of the School, including statistics re: staff, applicants, admissions, and graduations; developments in the School’s curriculum and facilities; scholarships and awards; and information about staff appointments.

Series consists of reports prepared by the Superintendent/ Director of the TGH School of Nursing and her assistants. Reports were prepared for various purposes and submitted to various individuals and corporate bodies: see sub-series-level descriptions for details. Reports contain information pertinent to the administration of the School. Some reports contain information relating to individual students of the School.

Series consists of minutes of the Toronto Western Hospital’s Board of Governors (1896-1967)/ Board of Trustees (1968-1986) which had been kept as seven volumes, six of which were locked binders. Some minutes from the Building Committee, Finance Committee, Executive Committee, Joint Conference Committee and other special committees are included. Minutes in each volume or file are in reverse chronological order.

Sous-fonds consists of student records created and maintained by the Atkinson School of Nursing of the Toronto Western Hospital and subsequently maintained by the TTH Nursing Education and Research Department. See series-level description for details. Sous-fonds includes one series:

1.2.1 Student records

Series consists of the permanent student academic records files created and maintained by the Atkinson School of Nursing of the Toronto Western Hospital. Records contained in individual files vary according to the date of creation, but may include student history cards containing grades, instructors’ assessments, dates of admission to the School and completion of course; certificates of preliminary education (high school academic records); reports from affiliated Hospitals, schools of nursing, and/ or education programs which students attended to receive specialized training and experience; and medical records. Some files also contain materials subsequently added, including letters from students or their agents requesting letters of reference for employment or information contained in their files, applications for employment in the Nursing Department of the TWH and employment records. For graduating years after 1920, only a 10% sample has been retained.

Sub-series contains the records of all students in each graduating class 1906-1920. File 1.2.1.1.156 contains a list of graduates 1898-1922.

Sub-series consists of monthly reports prepared by the Superintendent/ Director of the TGH School of Nursing and submitted to the TGH Superintendent/ Executive Director, beginning in September 1905 and ending in August 1973. Reports for 1960 to June 1970 and for September to December 1970 are missing.

Reports contain general information pertinent to the administration of the School, including statistics re: staff, applicants, admissions, and graduations; developments in the School’s curriculum and facilities; scholarships and awards; and information about staff appointments. Some reports also contain information about individual students, including information pertaining to academic performance, days lost to illness, infractions against School regulations and penalties imposed by School administrators.

Reports for 1905 to 1947, and 1970-1973 focus exclusively on the School of Nursing. Reports for 1948 to 1959 also contain information pertaining to the administration of the TGH Department of Nursing, including lists of departmental/ward head nurses (1951-1959) and recovery room reports (1952-1959).

Series consists of the permanent student academic record files created and maintained by the TGH School of Nursing. Records contained in individual files vary according to the date of creation. Most files will include the student history card, which contains grades by term as well as by subject; hours of practical work by department; dates and duration of absenteeism along with a list of cause or diagnosis and whether the time was made up; date of birth; home address; religion; place of birth; nationality of parents; instructors’ assessments; and dates of admission to the School and completion of courses. Information about vacations or illnesses was collected for the purpose of calculating the number of days absent and ensuring that the time was made up. Some student history cards include an attached picture. Files may also contain certificates of preliminary education (high school academic records); reports including records for hours of nursing care from affiliated Hospitals (e.g. The Hospital for Sick Children or the Toronto Psychiatric Hospital), schools of nursing, and/ or education programs which students attended to receive specialized training and experience; copies of medical records; and photographs. Some files also contain materials subsequently added, including letters from students or their agents requesting letters of reference for employment or information contained in their files, the actual letters of reference, applications for employment in the Nursing Department of the TGH and employment records, newspaper clippings and general correspondence.

Sub-series 1.1.2.1 to 1.1.2.10 contain the records of all students in each graduating class 1911-1920. Sub-series 1.1.2.11 to 1.1.2.37 each contain a 10% sample of the records of students in post-1920 graduating classes.

Sub-series contains the records for all students from the graduating class of 1911.

Records contained in individual files vary according to the date of creation, but may include student history cards containing grades, instructors’ assessments, dates of admission to the School and completion of course; certificates of preliminary education (high school academic records); reports from affiliated Hospitals, schools of nursing, and/ or education programs which students attended to receive specialized training and experience; and medical records. Some files also contain materials subsequently added, including letters from students or their agents requesting letters of reference for employment or information contained in their files, applications for employment in the Nursing Department of the Grace Hospital and the Toronto Western Hospital and employment records.

File 1.3.1.44 contains miscellaneous records for students in graduating classes 1912-1923. File 1.3.1.45 contains a list of graduates of the School 1892-1928.
