Series consists of two separate accessions of material related to the Toronto General Hospital and accumulated by the Office of the Surgeon-in-Chief.

Files 6.1.1 to 6.1.2 consist of a half-filled bound volume of signatures spanning the time period of April 5, 1935 to January 16, 1959, and an accompanying 5 page description of its history by Ronald J. Baird, Professor of Surgery, University of Toronto, and Senior Cardiovascular Surgeon at The Toronto Hospital. The book of signatures is a combination visitors’ register/attendance book with groups of signatures that do not always indicate the occasion. Some of the occasions noted include post-graduate courses and club meetings, such as for the Interurban Orthopaedic Club, McGill Toronto Surgical Club, and the Cardiovascular Surgeons Club. Notable signatories include Evarts A. Graham, Charles H. Best, Gordon Murray, Frederick G. Banting, Wilder Penfield, and Norman Bethune. Regarding the first pages of this volume, Dr. Baird remarks in his description, “what a galaxie [sic] of future stars in Toronto Surgery!” Dr. Baird’s description gives extensive details as to how the volume came to be in his possession before being passed onto Surgeon-in-Chief, Dr. Paul Walker.

Files 6.1.3 to 6.1.5 consist of various agreements and indentures that were originally kept in one file folder. The file had originally been in the possession of Dr. Alan Hudson, McCutcheon Chair and Surgeon-in-Chief at The Toronto Hospital from 1989-1991, and President and Chief Executive Officer, from 1991-2000. The file had been passed onto his successor Dr. Paul Walker, Vice President Surgical Directorate and Surgeon-in-Chief of The Toronto Hospital during a transfer of office either when Dr. Hudson became President and Chief Executive Officer (1991) or when Dr. Paul Walker resigned as the James McCutcheon Chair and Surgeon-in-Chief (March 31, 1999) to become Vice-President of the Toronto General Hospital (April 1, 1999). Subsequently the file came to be held by Dr. Walker’s successor as McCutcheon Chair in Surgery, Surgeon-in-Chief & Director of Surgical Services at University Health Network, Dr. Bryce Taylor (April 1, 1999- ).

Patient admission ledgers

Series consists of a bound volume of the Daily Record of the Operations of the Toronto Western Hospital from 1962-1981 and patient admission ledgers, also from the Toronto Western Hospital. Daily Records of Operations of the Toronto Western Hospital include the numbers of inpatients (adult male, adult female and infants), numbers admitted or births during the day, numbers of discharged during the day and numbers of deaths. The earlier patient admission ledgers include names of patients, their procedures, names of the surgeons and anaesthetists, and clinical information such as blood pressure. Later patient admission ledgers include registration numbers, name of patient, age, sex, date of admission and name of attending physician. File 5.5.8 is a computer printout that includes the additional information of medical record number and admitting diagnosis.

Series consists of 8 binders of course curriculum and 1 record book, which were maintained by the Department of Nursing Education along with student transcripts. File 1.1.13.1 consists of an oversized record book containing the examination grades for all students from the class of 1961 until the class of 1970, organized by class and then by courses within each year. The examination records for the Hospital for Sick Children Medical Surgical Nursing Lectures and Hospital for Sick Children Obstetrical Nursing course, for the same time period, are also included in this volume. Courses for which curriculum is provided include Philosophy of Nursing, Nursing I-III, Paediatric Nursing, and Psychiatric Nursing. Some of the binders contain a copy of the final examination, and class lists including individual addresses and marks. A more extensive course and curriculum list is provided for years 1970-1974.

File 1.1.11.1 contains negatives of graduating class photographs 1888-1974: negatives for 1889, 1892 to 1899 are missing. File 1.1.11.9 contains portraits of TGH School of Nursing Superintendents Mary Agnes Snively, Jean I. Gunn, and Mary E. Macfarland.

Series consists of photographs taken and collected by the Department during the time of David Allen as part of the Public Relations Department’s Photo and Slide Library. Files include photographs of Administration (former and current) of The Toronto Hospital, Trustees, Auxiliary, Dignitaries and Celebrities, Medical Staff, Residents, and so-called “historical archive files” referring to photographs of buildings, sites and past events. Some of the files also include newspaper clippings, enlargement and usage notations, speeches or programs, cataloguing information, consent forms, and mock-ups or samples of publications for which the photographs were used. Photographs were used for publications and other activities of this Department. Some of the material may also have been produced or used by IMS Creative Communications, a sub-department reporting to the Toronto General Hospital’s Public Relations Department.

Original consent forms are not part of this series. The original file list can be found in file 2.4.1. Subject files are arranged alphabetically. Staff and students are listed by their surname and then their given name. Headings have been included in the file list to indicate index tabs. Handwritten changes and additions to original file titles have been included as part of the file title.

File 2.4.52 includes correspondence from A. Hudson.

File 2.4.78 is of a patient who lived in the Toronto General Hospital for approximately 30 years of her life.

File 2.4.146 includes a ribbon-cutting photograph from the official opening of the Fraser M Fell Pavilion.

File 2.4.228 includes a photograph of American actor Mary Tyler Moore.

File 2.4.641 includes the OCI news release from 1985 regarding the Zeiss EM902 electron microscope and images of Dr. Ottensmeyer.

File 2.4.716 includes images of house staff; residents; Drs. R.M. Janes, Banting, Best, C.L. Starr, Morley, Botterell, Aikens, Beaumont, and Gordon Richards; Louise Eastwood, Charles O’Reilly, Mary Agnes Snively, Miss Balmer, Rev. John D. File 8.8.3 includes a report titled “Linking the Senior Management Team to the Hospital Mission: Inter-Related Roles and Responsibilities, April 1, 1993” which contains an organizational chart dated April 1, 1993. File 8.8.4 consists of reports from the Priority Programs Directorate, the Surgical Directorate, and the Professional/Diagnostics Services Directorate. The official minutes for these years have not been transferred to the Archives.

File 1.1.5.2 is a register of graduates of the School 1883-1940, including maiden and married names, whether deceased, and year of graduation. Files 1.1.5.3 and 1.1.5.4 are registers of student grades 1898-1920 and 1920-1935, including students’ and examiners’ names; date; course name; grade. File 1.1.5.5 is a register of students’ attendance 1927-1936. Files 1.1.5.6 and 1.1.5.7 are registers of probationary students 1913-1925 and 1926-1943, including students’ names, age, address; dates of probationary and full admission to the School; notes on students’ health, character, academic performance and potential. Sub-series consists of reports prepared by the Director of the TGH School of Nursing and submitted monthly (May 1946-1958), then quarterly (1959-1962) to the Ontario Department of Health.

Reports contain information about registered nurses employed at the TGH and about student nurses enrolled in the School; information about recent changes (appointments, resignations etc.) in the School’s administration and teaching staff and in the Hospital’s Department of Nursing; and information about students, including dates of birth, student illnesses and lists of recent withdrawals, graduations, and admissions.

Series consists of the permanent student academic record files created and maintained by the TGH School of Nursing. Records contained in individual files vary according to the date of creation. Most files will include the student history card, which contains grades by term as well as by subject; hours of practical work by department; dates and duration of absenteeism along with a list of cause or diagnosis and whether the time was made up; date of birth; home address; religion; place of birth; nationality of parents; instructors’ assessments; and dates of admission to the School and completion of courses. Information about vacations or illnesses was collected for the purpose of calculating the number of days absent and ensuring that the time was made up. Some student history cards include an attached picture. Files may also contain certificates of preliminary education (high school academic records); reports including records for hours of nursing care from affiliated Hospitals (e.g. The Hospital for Sick Children or the Toronto Psychiatric Hospital), schools of nursing, and/ or education programs which students attended to receive specialized training and experience; copies of medical records; and photographs. Some files also contain materials subsequently added, including letters from students or their agents requesting letters of reference for employment or information contained in their files, the actual letters of reference, applications for employment in the Nursing Department of the TGH and employment records, newspaper clippings and general correspondence.

Series consists of yearbooks created by graduating students of the TGH School of Nursing for 1923, 1925, 1931, 1954, 1955, 1959, 1967, 1968, 1970. Yearbooks contain text and photographs depicting graduating and continuing students, School faculty and staff, and student academic and social activities.

File 1.1.12.10 includes the yearbook belonging to Eugenia Wright with student autographs, as well as the announcement after-the-fact of the Commencement Exercises of the Forty-ninth Graduating Class of the School for Nurses for June 1931, and a poem entitled: <i>1931—T.G.H.—1981 / I’m Fine ……How About You?</i>
